MAIB issues safety bulletin

The UK’s Marine Accident Investigation Branch (MAIB) has issued a Safety Bulletin to highlight urgent safety lessons that have arisen in the early stages of its investigation into the mooring line failure on board the LNG carrier Zarga while alongside the South Hook LNG terminal, Milford Haven, UK, on 2 March 2015.
The failure resulted in a deck officer on board the LNG carrier suffering severe head injuries when he was struck by the mooring line.
The bulletin contains background, details of the mooring lines and winches in use on board the LNG carrier, initial findings and safety lessons.
Following the accident, the MAIB commissioned a series of tests and trials designed to measure the elongation and snap-back characteristics of the mooring lines used on board Zarga. A series of short video clips of these trials are available on the MAIB’s website.
